Terms and Conditions

By submitting an application to participate in the BLUEMOTH Experience Box Trial Program, you acknowledge and agree to the following terms and conditions:

1. Application Completion Requirements

Submission of an application form alone does not constitute a complete application.
To be considered for evaluation, all applicants must provide a valid audiogram completed within the past three (3) years. Applications will not be reviewed or considered complete until this requirement is fulfilled. 

2. Eligibility & Minimum Requirements

To be considered as a candidate, applicants must meet all of the following:

  • Be a previous hearing aid user

  • Provide a qualifying audiogram (within the last 3 years)

  • Reside within BLUEMOTH’s serviceable areas (excludes Florida and Wisconsin)

Meeting these minimum requirements does not guarantee selection.

3. Screening & Selection Process

All applicants are subject to a screening and selection process at the sole discretion of BLUEMOTH. Submission of an application does not guarantee participation. BLUEMOTH reserves the right to accept or decline any applicant for any reason. If an applicant is not selected, BLUEMOTH is not obligated to disclose the reason(s) for ineligibility. Qualified applicants may be required to complete a phone screening with the BLUEMOTH team and additional evaluation steps as determined by BLUEMOTH.

4. Participation Requirements

If selected as a participant, you agree to:

  • Provide accurate shipping information for home trial equipment

  • Maintain a valid payment method on file

  • Sign a payment authorization agreement prior to shipment 

Participants are fully responsible for all trial equipment provided. This includes returning all equipment within the designated timeframe and covering any costs associated with lost, unreturned, or damaged equipment. 

The home trial is 10 days long and begins with the scheduled unboxing appointment. At the conclusion of the home trial will be a scheduled evaluation appointment with a BLUEMOTH Audiologist. 

Participants are required to use devices as instructed during the home trial period, maintain responsiveness to BLUEMOTH team communication, and attend their scheduled BLUEMOTH appointments. Participants will be required to attend an unboxing appointment at the start of the home trial and an evaluation appointment at the end of the home trial. 

BLUEMOTH reserves the right to revoke participation at any time. Participation may be revoked for non-compliance, misuse of equipment, failure to communicate with BLUEMOTH staff, or any other reason at BLUEMOTH’s discretion. BLUEMOTH is not obligated to disclose the reason(s) for revoking participation.

5. Program Completion

Participants are required to complete all exit survey materials provided by BLUEMOTH. Participants are required to attend an end of trial evaluation appointment with a BLUEMOTH Audiologist upon completion of the home trial. Failure to complete required materials may impact eligibility for future programs. Upon trial completion, all trial equipment must be returned to BLUEMOTH using the included pre-paid return shipping materials and instructions.

6. Post-Trial Communication, Reviews & Content

You acknowledge and agree that BLUEMOTH may contact you after completion of your trial experience for purposes including, but not limited to collecting feedback and outcome information, requesting reviews or testimonials, or requesting participation in case studies or follow-up discussions. BLUEMOTH may also request user-generated content (UGC), including photos, videos, or written testimonials related to your experience. Participation in reviews, testimonials, or UGC requests is completely voluntary. Any content you choose to provide may be used by BLUEMOTH for marketing, educational, or promotional purposes. Where applicable, content will be used with your consent and anonymized or attributed based on your preference and approval.

6. Data Usage & Privacy

By participating, you agree that data collected from application submissions, screening, trial usage, and exit surveys may be used by BLUEMOTH. Additionally, anonymized data may be shared with BLUEMOTH’s partners at BLUEMOTH’s discretion.

7. Consent to be Contacted

By submitting an application to the BLUEMOTH Experience Box Trial Program, you expressly consent to be contacted by BLUEMOTH and its representatives for purposes related to your application and participation in the program. This includes communication via phone calls, email, and SMS. These communications may include, but are not limited to: application status updates, screening coordination and scheduling, trial participation instructions, follow-up communications and feedback requests. BLUEMOTH will use reasonable efforts to ensure communications are relevant to your participation and will not sell or distribute your contact information to third parties for unrelated marketing purposes.

8. Telehealth Terms & Consent

By participating in the BLUEMOTH Experience Box Trial Program, you acknowledge and agree that services will be delivered via telehealth, including phone calls, video conferencing, and other remote communication technologies. Telehealth interactions may include remote consultations and screenings, device setup guidance and support, counseling and follow-up discussions. By proceeding, you voluntarily consent to engage in telehealth communications with BLUEMOTH. You understand that telehealth services are intended to support your trial experience and are not a substitute for in-person medical evaluation, diagnosis, or treatment.
